You can download a copy of the form from our website at www.unitedwayswmn.org. “While the overall Dolly Parton Imagination Library website currently offers the ability to select the language most applicable to the user; the majority of local families are registering through our website or by completing a registration form available at one of our local hospitals, health care providers, ECFE programs, public health offices, etc. said executive director, Marcy Heemeyer. As the local affiliate and financial sponsor for the region, we hope to further expand registration language offerings to ensure simple sign-up for all members of our diverse communities.”
